Images in anesthesiology: facemask ventilation with a frontonasal encephalocele.

چکیده
698 March 2015 A full-term, 69-dayold infant with an antenatal diagnosis of a frontonasal encephalocele presented for a bifrontal craniotomy and encephalocele resection. The encephalocele measured 4.5 cm by 4.5 cm and extended from the patient’s glabella to his nasal alae. It was soft and covered with intact skin. The remainder of his airway examination (including computed tomography) was unremarkable. The location of the encephalocele precluded routine facemask ventilation. Seventy-five percent of encephaloceles arise from the occiput and may be associated with challenging direct laryngoscopy, whereas frontonasal and frontoethmoidal encephaloceles may be difficult to mask ventilate in up to 19% of patients.1–3 We knew that the unique nasal structures encountered in veterinary medicine necessitate an anesthetic mask that encompasses a majority of the animal’s face. Therefore, we placed an adult mask (size 5) over the awake patient’s entire face, incorporating the encephalocele (fig.). With minimal irritation to the nonsedated infant, we were able to assist with and assume ventilation. The facemask was rotated 180 degree in an attempt to mirror the contour of the skull and seal along the frontal bone, zygomatic bones, and mandible. No direct pressure was applied to the globes. Inhalational induction and subsequent direct laryngoscopy were uneventful.

Prenatal diagnosis of frontonasal dysplasia with anterior encephalocele.
Frontonasal dysplasia is a rare congenital anomaly affecting the eyes, nose and forehead, and occurs sporadically in most of the cases. A 24-year-old woman was referred to our unit at 27 weeks gestation due to the preliminary diagnosis of encephalocele. The sagittal and axial sonography of the fetal face depicted a midline mass measuring 3.8 × 4.2 cm, projecting anteriorly between the fetal orb...
متن کاملEffects of muscle relaxants on mask ventilation in anesthetized persons with normal upper airway anatomy.
BACKGROUND Recent studies suggest advantages of muscle relaxants for facemask ventilation. However, direct effects of muscle relaxants on mask ventilation remain unclear because these studies did not control mechanical factors influencing ventilation. We tested a hypothesis that muscle relaxants, either rocuronium or succinylcholine, improve mask ventilation. METHODS In anesthetized adult per...

We report ten sporadic cases of Brazilian patients with facial midline defects, callosal agenesis, basal encephalocele, and ocular anomalies. This very rare cluster of anomalies has been well reported before. However, only until recently it is recognized as a syndrome belonging to frontonasal dysplasia spectrum. The ten cases confirm a distinct clinical entity and help to define the phenotype m...

291 Dear Editor-in-Chief, We have read the paper of Song et al. [1] on frontonasal dysplasia deformity. It was an interesting discussion of an image that depicts a boy with a nose deformity, and it was reported that no basal encephalocele was observed. We would like to point out that we had a similar case of a girl with the same nose deformity. As in the report by Song et al., our patient also ...

AIMS To document the optic disc abnormalities in patients with frontonasal dysplasia in association with basal encephalocele. METHODS Names and hospital numbers of patients with midline clefts were obtained from the ophthalmology and genetics database. Six patients were identified who had the following common findings: midline facial cleft with midline cleft lip and palate; hypertelorism; abs...
